Music Instructor Ticket Request Form

Beginning Fall 2017, Friends of Music will subsidize the ticket price for off-campus concerts for groups of music students in academic and performance classes as requested by their music instructors. The instructor must make the request using this form at least 3 weeks before the concert date. In cases where an instructor pays for the tickets and asks for reimbursement, this form must still be filled out and approved three weeks in advance. We will fulfill concert ticket subsidy requests within the limits of our budget.
